Because this system is shared by both waste removal and reproduction, the Bulbourethral or “Cowper’s” Gland works during sexual activity to remove any urine from the urethra by secreting an alkaline fluid.

With so many factors at play, Dr. Paul Edward Perito MD points out a variety of things that could malfunction. A urinary tract infection, common among members of both sexes, is caused by bacteria growing in the kidney, the bladder, or the tube that carries urine through the body, Dr. Paul Edward Perito MD says. UTIs can be caused by sexual activity, Dr. Paul Edward Perito MD explains, but this is more common in women than men. UTIs are usually treated with antibiotics, but mild cases can be counteracted by drinking unsweetened cranberry juice and vast amounts of fluids, especially water.

Kidney stones aren’t uncommon, especially among men, Dr. Paul Edward Perito MD notes. Most frequently caused by a lack of enough water in the body, kidney stones can be excruciating for patients and in some men is a recurring problem. According to Dr. Paul Edward Perito MD, incontinence is another urologic problem, generally seen more in men who have had prostate surgery and women with a history of vaginal births. Prostate enlargement can also cause difficulty in urinating for men, Dr. Paul Edward Perito MD states.

For men suffering infertility, Dr. Paul Edward Perito MD points to veins that drain the testicles as the most common cause. These veins, called varicocele, can either dilate or swell, causing the temperature of the testicles to escalate. Low testosterone and ingestion of alcohol and certain medications can also be blamed, Dr. Paul Edward Perito MD says.

For men suffering erectile dysfunction, sometimes the causes can be psychological. But as Dr. Paul Edward Perito MD points out, smoking and heart disease can create problems with the vascular and arterial systems, resulting in a loss of sexual function.

Presentation Solutions: Thank you for joining us today, Dr. Paul E. Perito. How does a patient know if he’s suffering from erectile dysfunction?

Dr. Paul E. Perito: Thank you for having me. If a man is having difficulty developing and maintaining an erection during sexual intercourse, it may be time to seek treatment.

Presentation Solutions: But there are varying degrees of erectile dysfunction, right, Dr. Paul E. Perito?

Dr. Paul E. Perito: In mild cases, a patient is able to attain an erection most of the time, say seven or eight times for every ten attempts. These cases may be treatable through medication or injections. A moderate case would be a patient who is successful about half the time, with a severe case being a patient who can achieve erection less than four attempts out of ten. Some severe patients are never able to achieve erection.

Presentation Solutions: How common is sexual dysfunction?

Dr. Paul E. Perito: It’s more common than most people realize. According to the National Institutes of Health, more than 30 million men in this country suffer from the condition, with it growing more likely with age. Almost half of all men older than 75 suffer from erectile dysfunction.

Presentation Solutions: Those are some pretty grim statistics, Dr. Paul E. Perito.

Dr. Paul E. Perito: Yes, but it’s important to note that no matter what a man’s age, erectile dysfunction can be treated.

Presentation Solutions: Dr. Paul E. Perito, are these statistics increasing as baby boomers age?

Dr. Paul E. Perito: Actually, yes. The number of patients seeking treatment is said to be double what it was ten years ago. As the demand increases, we’re trying to find new, less painful ways to treat patients, with shorter recovery time.

Presentation Solutions: What causes erectile dysfunction, Dr. Paul E. Perito?

Dr. Paul E. Perito:: Patients with diabetes often develop erectile dysfunction—in fact, statistically about half of all diabetes patients will suffer from erectile dysfunction within ten years of diagnosis. Smoking also seems to be connected to the problem, with the problem more prevalent among men who smoke more than a pack a day. And, of course, the likelihood of erectile dysfunction increases with age.

Presentation Solutions: Dr. Paul E. Perito, can certain medications cause erectile dysfunction?

Dr. Paul E. Perito:: Erectile dysfunction is a side effect of some IMISFC blood pressure medicine and some sedatives. Narcotics and alcohol can also be contributing factors. In these cases, we may recommend the patient discuss adjusting medications with his patient or refrain from drinking for a while to see if the problem clears up.

Dr. Paul Perito: Not a problem. Erectile Dysfunction is a matter that needs an open discussion every once in a while.

Presentation Solutions: Can you give us an idea about what makes your way of performing a penile implant unique?

Dr. Paul Perito: One of the major differences is that I use an artificial erection; this offers me plenty of corpora to negotiate when I am placing stay sutures.

Presentation Solutions: And that is important?

Dr. Paul Perito: Yes, in the thousands of case I have done, I’ve never seen a case of glandular hypoesthesia or anesthesia. I believe this is because you can avoid the dorsal nerves with an artificial erection.

Presentation Solutions: Are there other differences?

Dr. Paul Perito: Many of the duplicitous steps have been eliminated allowing the case to move along thus diminishing tissue damage while lowering operative times.

Presentation Solutions: Does the penis have to be dilated to insert the implant?

Dr. Paul Perito: I don’t have to use dilation, the artificial erection accomplishes that; I believe that by not damaging the tissue with serial dilation that patients end up with more sensate glands after the procedure.

Presentation Solutions: We’ve heard that you place your reservoir in a different location than other surgeons?

Dr. Paul Perito: Yes. Traditionally a surgeon will place the reservoir tank in the behind the pubic bone. I use the area behind the abdominal wall or within the abdominal wall.

Presentation Solutions: Does that make the procedure safer?

Dr. Paul Perito: In my opinion, this technique keeps the patient safe from catastrophic bowel, bladder or vascular injuries. This is especially important in patients who have undergone previous radical prostatectomies.

Presentation Solutions: It all sounds so complicated. Does the surgery take long?

Dr. Paul Perito: I’ve completed the procedure in as little as 6 minutes. It is not a time intensive surgery.

Presentation Solutions: And what about recovery time?

Dr. Paul Perito: Most men report full sexual function in 4 weeks. The remaining cases report a 6 week wait time.

In her three decades of working with women, Marla Ahlgrimm, founder of Women’s Health America, has found that exercise makes a big difference in a woman’s health. But for many women, “exercise” is a bad word. How can women find an exercise program that they’ll want to stick with for life?

To begin, Marla Ahlgrimm, R.Ph. stresses the importance of asking oneself why you are interested in beginning a new exercise program. If you are hoping to stay healthy, you will likely have more success than if your only goal is to lose weight. Likewise, if your goal is to do something you enjoy doing, and exercise is one of those things, you’re more likely to stick with it for many years, Marla Ahlgrimm says.

According to Marla Ahlgrimm, co-founder of Madison Pharmacy Associates, the key is to focus on physical activities you enjoy doing. Many women give up when an exercise program doesn’t yield results immediately, Marla Ahlgrimm, R.Ph. points out. By choosing physical activities you enjoy doing, you’ll stick with it long enough to begin seeing and feeling overall improvements.

Marla Ahlgrimm advises several small changes you can make that will improve your overall health. For good health, Marla Ahlgrimm, co-founder of Madison Pharmacy Associates, states that you should incorporate 20 to 30 minutes of aerobic activity at your target heart rate at least three days a week. By spreading these activities over the day, Marla Ahlgrimm suggests, you can work in thirty minutes of moderate exercise without having to set thirty minutes aside to do a formal workout. This can be done by using stairs instead of elevators, as well as parking far away and walking to your destination at a brisk pace. You can also spread your thirty minutes out throughout the day by dancing, riding an exercise bike, or playing actively with your children for a few minutes throughout the day.

To be most effective, your exercise routine should utilize the large muscles of your lower body, according to Marla Ahlgrimm, founder of Women’s Health America. Activities that fit in that category include walking, jogging, cycling, cross-country skiing, stationary cycling, and aerobic dance classes, Marla Ahlgrimm states.

Additionally, Marla Ahlgrimm recommends weight-bearing exercises. This includes jogging and dancing, Marla Ahlgrimm points out, but not swimming. And pharmacist Marla Ahlgrimm advises against working one part of the body in the hopes of losing inches there. Marla Ahlgrimm points out that by focusing on making all of your body’s muscles stronger, you’ll slightly increase your metabolism to maintain weight, decrease fluid retention and improve your lean muscle mass. Your body will become more efficient, Marla Ahlgrimm, R.Ph. says, and you’ll increase the strength of your body’s joints and bones and improve balance which is much more important as you get older.

You should begin any exercise program gradually, Marla Ahlgrimm advises, to avoid injury. While your cardiovascular system may be able to adapt, your body’s musculoskeletal systems may break down if pushed too hard, Marla Ahlgrimm, pharmacist, warns.

The benefits of an exercise program go well beyond stronger muscles and joints, Marla Ahlgrimm states. Both women and men report regular exercise helps them sleep better at night and gives them a greater feeling of energy throughout the day. Exercise also helps them better cope with stress, Marla Ahlgrimm adds, providing an outlet for that ‘fight or flight’ physical response brought on by stress.

Lastly, if your exercise program makes you feel overly tired, Marla Ahlgrimm suggests pulling back a little. Decrease the intensity or choose different exercises until your body adjusts.

Advice from Dr. Naveed Fazlani: How to Know if You Have Sleep Apnea

Have you ever been in the room with a person that snores? Not just any snore, but the kind of snore that rattles the walls? A common myth, Dr. Naveed Fazlani has found, is that snoring is a sign of sleep apnea. But Dr. Fazlani is quick to point out to his patients that not everyone who snores has sleep apnea. Conversely, he adds, not everyone who has sleep apnea snores.

The misconception stems from the fact that snoring is often the issue that brings a sleep apnea patient into the office for a sleep study, Dr. Naveed Fazlani states. In fact, bed partners are often the ones who uncover a patient’s sleep apnea, he has found, as patients often fall back to sleep so quickly after waking that they do not realize they have the sleep disorder. He explains that sleep apnea causes a patient to wake up multiple times throughout the night with an inability to breathe or a choking feeling. This may or may not be accompanied by snoring loud enough to be disruptive to a bed companion, Dr. Fazlani says.

In order to learn how to spot sleep apnea, Dr. Fazlani believes the first step is to understand what causes the condition. He explains that sleep apnea occurs when the individual’s airflow is obstructed by having a small upper airway. This causes the patient to wake up, catch his or her breath, then generally fall right back to sleep.  According to Dr. Naveed Fazlani, because many sufferers fall asleep so quickly, they often have no idea they woke up numerous times during the night.

If a patient lives alone or cohabitants are heavy sleepers, sleep apnea may go undetected. This can be dangerous, Dr. Naveed Fazlani says, as the breathing obstruction causes long-term health impairments in addition to the daily dangers of loss of sleep. According to Dr. Fazlani, health risks can include heart problems, high blood pressure, and an increased risk of stroke. He explains this is thought to be because of the changes in heart rate and blood pressure that occur during sleep.

Feeling fatigued during the day could be an indication that you have sleep apnea, adds Dr. Naveed Fazlani, although fatigue can be traced back to a number of other issues, as well. Other symptoms he has seen include morning headaches, a feeling of dry mouth, sore throat, and awakening frequently during the night to urinate. If you are aware of waking up during the night feeling as though you are choking, Dr. Fazlani warns, this is definitely something to discuss with your physician.

How Dr. William Howland Treats Children with Allergies

In his years of practice, Dr. William Howland has seen numerous children suffering from allergies and asthma. Dr. William Howland knows how difficult it is for parents to watch their children struggle with allergies and strives to help children through his practice and his medical research company, Sirius Clinical Research.

Dr. William Howland blames the increase in child allergies on a couple of things. First, Dr. William Howland cites the hygiene hypothesis, which states that our modern world has become so hygienic that our immune systems are not exposed to enough infections to strengthen them. Another reason for the increase in childhood allergies and asthma, Dr. William Howland believes, is genetics. Most of the children Dr. William Howland sees have at least one parent with allergy problems as well.

When Dr. William Howland sees children, it is generally to treat one of five conditions prevalent in childhood allergies. One of those five issues, Dr. William Howland says, eczema.  Dry, red, itchy skin is often the first sign of allergies in an infant. Good skin care measures using mild soaps and moisturizers are the first treatments.  Rarely eczema is a sign of food allergy and sometimes allergy testing is helpful to guide dietary changes.

The second allergy problem that develops early in childhood is hay fever. With symptoms such as runny nose and itchiness, hay fever tends to be more unpleasant than life threatening, Dr. William Howland says.  Though not life threatening, significant hay fever can cause sleeping difficulties, daytime drowsiness, and interfere with learning and memory.

On the other hand, Dr. William Howland says asthma can be much more serious. Without speedy treatment, asthma can result in hospitalization, permanent scarring of airways, and very rarely even death. Luckily, medications are now available to get childhood asthma under control. The key is early diagnosis.

A fourth childhood allergy Dr. William Howland often sees is childhood food allergies. According to Dr. William Howland, most children are allergic to one of five foods: peanuts, soy, milk, egg and wheat. In most cases, Dr. William Howland alters the diet of a child, but he also provides an EpiPen (autoinjectable epinephrine which can stop and reverse a food reaction) for parents to keep on hand at all times.

Sinus disease is the fifth condition Dr. William Howland’s office most often treats in children. In Texas, trees and grasses are the leading cause of spring allergies, explains Austin-based allergist William Howland MD. In Austin, the heaviest pollinating trees are oaks. Ash and elm trees pollinate in mid-February, reports Dr. Howland. In March, oak pollination starts and levels peak around April 1st. Cottonwood and pecan trees are the last to pollinate in May, states William Howland MD.

Oak trees become covered with green and yellow catkins. On windy days they burst and spread a large amounts of pollen, says William Howland MD. The catkins cover the ground, sidewalks, yards, streets and cars in mid-April. However, sweeping the catkins spreads and releases more pollen, cautions William Howland MD.

William Howland MD explains that in Austin, commonly pollinating grass varieties are rye, Johnson and Bermuda. Unfortunately, if a person is allergic to rye grass they are allergic to all grasses, states William Howland MD. Rye grass tends be located on lawns and in agricultural areas. Johnson grass is usually found on pastures and right-of-way areas and along stream banks. However, Bermuda grass is a perennial that is difficult to escape. It grows in such varied places as coastal areas, golf courses, lawns, parks, and recreation areas and on sports fields, explains William Howland MD.

Like all pollen allergies, people who are allergic to grass tend to suffer from hay fever, says William Howland MD. Typically, people with hay fever are congested, sneeze and have itchy eyes. The symptoms aren’t often as severe as they are for a ragweed or tree allergy, because grass pollen counts normally aren’t as high in Central Texas, explains William Howland MD. However, grasses may pollinate from March to November depending on rainfall during the summer, so people with allergies suffer a little bit longer. William Howland MD suggests that a grass allergy can be minimized with prescription antihistamines and nasal sprays. You can even start prescription allergy-preventing nasal sprays before the allergy season starts, says William Howland MD.

William Howland MD suggests that people who are sensitive to pollen should run the air-conditioning instead of having their windows open in the house or in a vehicle, if possible. Another way for people to protect themselves is to limit exposure outside, William Howland MD points out. Peak pollen hours are between 5 a.m. and 10 a.m., and people who suffer from allergies should avoid outside activity during that time. William Howland MD also suggests that patients don’t hang clothing out to dry during grass pollen season, as it could end up on bedding.
